For COCC students and faculty:

Interlibrary Loan Form - use this form to request books, articles, etc. that are not available through our library or Summit.

For COCC and OSU-C faculty:

COCC Faculty Book Order Form - to be used by COCC faculty who wish to place library purchase orders.

Faculty Non Print Hold Request - to be used by instructors for requesting holds on nonprint materials (videotapes, DVDs, etc.) for classroom use.

Faculty Reserves Form - to be used by instructors for placing items on library reserves.
